Michelle Ortiz (Case Manager)

My responsibilities are to ensure that our clients are connected to mainstream services through the case work services that we provide.We determine whether the support provided will be low or medium intensity. Clients that require low-intensity support will have the knowledge and skills required to integrate within the community and connect to Government agencies with ease. Clients who require medium-intensity support will undertake a needs assessment and be assigned a case worker to monitor them with ongoing support. Case managers will determine whether a client will need a case plan – identifying actions, responsibilities and timeframes.
